A) 110 Minue St. Carteret, NJ 07008. The employer did not ensure that each employee engaged in commercial roofing activities was protected from falls by the use of a complete fall protection system. Employees were using body harness attached to a rope without the use of a lanyard. This condition exposed employees to falls of approx. 23 feet, 4 inches. Hazards noted on or about 05/13/2025
