On or about May 15, 2025, employees engaged in roofing operations located at 15920 Bauman Ave, Omaha, Nebraska 68116, were exposed to fall hazards in that employees were observed working at heights greater than 10 feet without any form of fall protection.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about May 15, 2025, employees engaged in roofing operations located at 15920 Bauman Ave, Omaha, Nebraska 68116, were exposed to fall hazards in that the employer failed to train his employees in the recognition of hazardous conditions on the job site as they relate to working from heights greater than 6 feet as well as on the inspection and subsequent use of fall protection equipment prior to having employees perform roofing work at heights where the fall distance ground to eave is greater than 10 feet.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about May 15, 2025, employees engaged in roofing operations located at 15920 Bauman Ave, Omaha, Nebraska 68116, were exposed fall hazards in that employees were using an aluminum extension ladder as means of access and egress to the roof and the extension ladder was not extend at least 3 feet above the upper landing surface. On this day the employer had the ladder extended approximately 18 inches above the eave denying the employees the necessary hand hold used in transitioning on and off the ladder.
