29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a) On or about May 15, 2025, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns, in that protective eyewear was not provided to employees engaged in framing activities while operating these tools at 130 PSI.

29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded:  a) Top plate, On or about May 15, 2025, and at times prior, the employer exposed employees to amputation/severe laceration hazards, in that the air compressor was missing a cover over the starter recoil, exposing operators to moving parts when starting and stopping the equipment.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a) Top plate: On or about May 15, 2025, and at times prior, the employer exposed employees to fall hazards of approximately 15 feet 3 inches to the lower level, in that a fall protection system was not provided to employees engaged in framing activities.

29 CFR  1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stair rail system along each unprotected side or edge:  a) Stairway, On or about May 15, 2025, and at times prior, the employer exposed employees to fall hazards of approximately 10 feet 2 inches to the lower level, in that the stairway used for access was not equipped with a handrail system along each unprotected side or edge, when employees were engaged in framing activities on the upper levels.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:   a) On or about May 15, 2025, and at times prior, the employer failed to inspect all electrical extension cords used to provide power to electric tools which employees engaged in framing operations were using, in that the extension cords powering the reciprocating and circular saws were damaged. An inspection of these cords was not performed prior to the shift as these cords were not repaired or removed from service.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a) On or about May 15, 2025, and at times prior, the employer exposed employees to 120V electrical shock hazards while operating reciprocating and circular saws, in that yellow and red extension cords were missing the grounding pin.
