(a)	On or about May 14, 2025, at the back wall of an two-story commercial new build car wash building, where employee was installing rebar from the second-tiered scaffold that was not fully planked.

29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  (a)	On or about May 14, 2025, employees doing  masonry work on a commercial new build car wash while working from a two-tier scaffold system where a ladder was not available for safe access.

On or about May 14, 2025, employees doing masonry work on a commercial new build car wash while working  and a piece of rebar came in contact to overhead power line.
