29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: a.) IBI/RR, LLC dba Rachel's Roadhouse/Mercer , kitchen line cooler - On or about 5/15/25, and at times prior thereto, employees were being shocked when touching the cooler and another metal surface. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.22(a)(3):29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. a.) IBI/RR, LLC dba Rachel's Roadhouse/Mercer; server area - On or about 5/15/25, and at times prior thereto, a bucket in placed on the floor in the middle of the server area to catch water leaking from the ceiling. The bucket creates a potential trip, fall hazard as employees work in that area to retrieve drinks and silverware. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Employees were being shocked when touching the dishwasher and another metal surface. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
