29 CFR 1926.1053(b)(1):  When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, 1821 Marmaduke Street, Pittsburgh, PA: On or about May 15, 2025 during roofing activities, the employer did not ensure a ladder, used to get up/down to and from the roof, was properly extended in that the ladder only extended approximately 18 inches above the roof line.

29 CFR 1926.20(b)(2):  A program was not implemented for frequent and regular inspections of the job sites, materials, and equipment to be made by a competent person:    a) Job site, 1821 Marmaduke Street, Pittsburgh, PA: On or about May 15, 2025, during roofing activities, the employer did not ensure that a system of hazard recognition and corrections was implemented at the job site. Deficiencies noted included, but were not limited to, failing to use fall protection on roofs or scaffolds, working near an electrical service drop without proper safety actions first being taken, not having a ladder properly extended above the roof line, allowing a section of scaffold footing to be set up in an unstable manner and not using safety glasses when working with nail guns.
