29 CFR 1926.20(b)(2): A program was not implemented for frequent and regular inspections of the job sites, materials, and equipment to be made by a competent person: a) Job site, 1821 Marmaduke Street, Pittsburgh, PA: On or about May 15, 2025, during roofing activities, the employer did not ensure that a system of hazard recognition and corrections was implemented at the job site. Deficiencies noted included, but were not limited to, failing to use fall protection on roofs or scaffolds, working near an electrical service drop without proper safety actions first being taken, not having a ladder properly extended above the roof line, allowing a section of scaffold footing to be set up in an unstable manner and not using safety glasses when working with nail guns.
