29 CFR 1910.23(b)(8): The employer did not ensure that ladders are used only for the purposes for which they were designed:  On or about May 8, 2025, it was documented that employees performing parts retrieval activities in the service bay of Bish's RV of Kearney located at 115 East 1st Street, Kearney, Nebraska were exposed to fall hazards in that employees accessed elevated areas using an A-frame ladder leaned in a collapsed configuration instead of the open position as designed and required by the manufacturer.

29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  On or about May 14, 2025, it was identified that employees engaged in recreational vehicle servicing and maintenance activities at BRV Kearney, LLC dba Bish's RV of Kearney located at 115 East 1st Street, Kearney, Nebraska were exposed to fall hazard in that employees working indoors atop recreational vehicles between 8 to 14 feet high were not protected from falls greater than 4 feet to a lower level.  This was most recently documented:  i.  On May 13, 2025 where two employees were making repairs to AC and applying sealer to the vent on the top of a camper in the shop ii. On May 12, 2025 where an employee was replacing a roof vent while standing on the top the camper in the shop.  BRV Lincoln, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard , which was contained in OSHA inspection number 1677238, Citation number 1, Item number 2 and was affirmed as a final order on November 13, 2023, with respect to a workplace located at 1000 and 1040 Saltillo Road, Nebraska.  Both BRV Lincoln, LLC and BRV Kearney, LLC operate under parent company Bish's RV Holdco, Inc.

29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v):  On or about May 14, 2025, it was observed that employees performing recreational vehicle servicing and maintenance in the service bays of BRV Kearney, LLC dba Bish's RV of Kearney located at 115 East 1st Street, Kearney, Nebraska were exposed to contact hazards in that employees transferred antifreeze into unlabeled/improperly labeled portable containers, where the portable containers were not immediately used by employees who transferred them.

29 CFR  1910.1200(g)(8):   The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s)  On or about May 14, 2025, it was observed that employees performing recreational vehicle servicing and maintenance in the service bays of BRV Kearney, LLC dba Bish's RV of Kearney located at 115 East 1st Street, Kearney, Nebraska were exposed to contact hazards in that the employer failed to maintain and make available Safety Data Sheets for: Champion RV & Marine Anti-freeze both are chemicals employees use in the maintenance and repairs they perform.
