On or about May 15, 2025, at an establishment located at 1133 East Grand Avenue in Rothschild, Wisconsin, in the service shop; the employer did not ensure that two 55-gallon steel drums containing non-chlorinated brake cleaner, a Category 1 flammable liquid, stored outside of an inside storage room or storage cabinet in the building or in any one fire area of the building did not exceed 25 gallons, exposing employees to fire and burn hazards. One of the steel drums was full and the other drum had approximately 15 gallons remaining inside.

Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), shall not be dispensed into containers unless the nozzle and container are electrically interconnected. Where the metallic floorplate on which the container stands while filling is electrically connected to the fill stem or where the fill stem is bonded to the container during filling operations by means of a bond wire, the provisions of this section shall be deemed to have been complied with.  On or about May 15, 2025, at an establishment located at 1133 East Grand Avenue in Rothschild, Wisconsin, in the service shop; the employer did not ensure the steel, 55-gallon drum containing non-chlorinated brake cleaner was grounded and bonded prior to employees pumping the category 1 flammable liquid into a plastic container, exposing employees to fire and burn hazards.

29 CFR  1910.132(d)(1):The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall: (i) Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment; (ii) Communicate selection decisions to each affected employee; and, (iii) Select PPE that properly fits each affected employee.  On or about May 15, 2025, at an establishment located at 1133 East Grand Avenue in Rothschild, Wisconsin, in the service shop; the employer did not assess the workplace to determine if hazards were present, or likely to be present, which necessitated the use of personal protective equipment (PPE) in an automotive repair and lubrication shop. Such PPE includes, but is not limited to, chemically resistant hand protection, head protection, arm protection, and eye protection, when employees are exposed to chemical absorption and struck-by hazards created by falling debris and fluids from the undercarriage of vehicles.

Lube Techs were exposed to eye and face irritation while spraying brake cleaner and draining fluids, and struck-by hazards to the face and eyes from falling debris while working under vehicles in the service pit.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  On or about May 15, 2025, at an establishment located at 1133 East Grand Avenue in Rothschild, Wisconsin, in the service shop; employees using non-chlorinated brake cleaner and brake fluid were exposed to corrosive splash hazards to the eyes where there were no suitable facilities for flushing of the eyes for immediate emergency use. The eyewash station located in the service shop was not capable of flushing the eyes for 15 minutes.
