29 CFR  1926.416(e)(1):Worn or frayed electric cords were used.  a) On or about May 16, 2025, at the above-addressed job site, extension cords that were used by employees to power hand tools had been damaged and lacked insulation.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(4)(i):  Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a)  On or about May 16, 2025, employees were exposed to falls of approximately 11 feet while working around unprotected floor openings.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation.  a) On or about May 16, 2025 at the above mentioned address,  employees were exposed to eye injury while operating pneumatic nail guns without the benefit of eye protection.   Buildmax Company Inc, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1652421, citation 1, item 1, and was affirmed final order on March 28, 2023, with respect to a workplace located at 317 Elm St. Park Ridge, IL.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were the employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a) On or about May 16, 2025, at the above addressed jobsite, employees were exposed to fall hazards of  approximately 10.7 feet ground to eave while performing residential roofing work.  Buildmax Company Inc, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1742089, citation 1, item 2, and was affirmed final order on January 21, 2025, with respect to a workplace located at 529 Franklin St. Downers Grove, IL.  Buildmax Company Inc, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1652421, citation 1, item 2, and was affirmed final order on March 28, 2023, with respect to a workplace located at 317 Elm St. Park Ridge, IL.
