On or about May 16, 2025, at the above addressed jobsite, employees engaged in roofing activities were not protected from fall hazards while working at an elevation of approximately eight (8) feet, above the ground, thereby exposing employees to fall hazards.  b.  On or about May 16, 2025, at the above addressed jobsite, employees engaged in roofing activities were not protected from fall hazards while working at an elevation of approximately eight (8) feet eight (8) inches above the ground, thereby exposing the employees to fall hazards.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a.  On or about May 16, 2025, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.
