29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill. a) Job Site: 190 3rd Street, Beaver, PA 15009: On or about May 15th, 2025 employees were not using fall protection as intended by manufacturer in that the fall protection harness was not connected with the "D" ring between the shoulder blades. One employee was working from a height of approximately 30 feet in order to install a rubber roof on a elevator shaft.
