29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. On or about May 16, 2025, and at times prior, on the west end of the OCAT 4 hanger project, employees conducted concrete pumping activities while allowing employees of a subcontractor to work around the concrete pumping unit where recognition on how to avoid unsafe conditions was not provided.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided. On or about May 16, 2025, and at times prior, on the west end of the OCAT 4 hanger project, employees conducted concrete pumping activities while employees of a subcontractor performed concrete finishing activities where proper protective shields and barriers were not provided to keep employees out of the danger zone.
