29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates and mud sills or other adequate firm foundation: a) On or about May 16, 2025, at location 301 NW 82nd Ave, Plantation, FL, 33324, on the east side of the medical establishment, the scaffold system lacked mud sills, exposing employees to a 16 feet fall, struck by and/or crushed by hazards.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section: a) On or about May 16, 2025, at location 301 NW 82nd Ave, Plantation, FL, 33324, on the east side of the medical establishment, employees were exposed to 16 feet fall hazard while standing on a scaffold platform without a guardrail system and means of fall protection.
