29 CFR  1910.178(m)(5)(iii):  Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have the load engaging means fully lowered, controls neutralized, and brakes set:   220 Whalon St, Fitchburg MA:  On or about May 16, 2025, a forklift operator exited the seat and stood on the right side of the operator cab to reach an overhead door, and the controls of the forklift were not neutralized and the brakes were not set.  As a result, upon attempting to remount the seat, the vehicle moved, resulting in the employee operator receiving fatal injuries.
