29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. At the Premo line, located at 313 SW Windswept Glen, Lake City, FL: On or about May 19, 2025, the employer exposed employees to amputation hazards, in that employees were exposed to in-going nip points and rotating parts not protected by a one or more methods of machine guarding.   b.  At the "VIVA" line, specifically at the  Sencorp 2500 Thermo Former, located at 313 SW Windswept Glen, Lake City, FL: On or about May 19, 2025, the employer exposed employees to crush, burns, and amputation hazards, in that interlocks of the gates were bypassed to allowed employees access the molding dies, cutting knife and printer head area inside the machine while running.
