29 CFR  1910.147(c)(4)(ii):The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (D)Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  a) On or about May 19, 2025, the employer did not ensure that the machine-specific lockout procedure for the Mazak Lathe, Work Center 5102R, contained the requirements of 1910.147(c)(4)(ii). The procedure did not include the intended use, steps to shut down and isolate pneumatic and electrical energy, or the type and placement of lockout devices, exposing employees to caught-in hazards while performing servicing tasks such as cleaning the lathe and greasing the chuck.  b) On or about May 19, 2025, the employer did not ensure that the machine-specific lockout procedure for the Kingston Lathe, Work Center 5111R, contained the requirements of 1910.147(c)(4)(ii). The procedure did not include the intended use, steps to shut down and isolate pneumatic and electrical energy, or the type and placement of lockout devices, exposing employees to caught-in and struck-by hazards while performing servicing and setup tasks such as cleaning, lubricating, and loading or removing parts.

29 CFR  1910.147(c)(7)(i)(A): Training and Communication. Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a) On or about May 19, 2025, the employer did not ensure that machine operators, including those using the Mazak Lathe (Work Center 5102R) and the Kingston Lathe (Work Center 5111R), received authorized employee training. Employees were not trained to recognize hazardous energy sources or the methods and means to control them prior to servicing tasks such as cleaning, lubricating, greasing, setup, and placing or removing parts. Employees did not receive the training necessary to identify hazardous energy sources, including electrical for the Kingston lathe and electrical and pneumatic for the Mazak lathe.

29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  (1) Preparation for shutdown. Before an authorized or affected employee turns off a machine or equipment, the authorized employee shall have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy. (2) Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. (3) Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). (4) Lockout or tagout device application. (5) Stored energy. (6) Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished.  a) On or about May 19, 2025, employees operating the Mazak Lathe Work Center 52102R were exposed to caught-in hazards, electrical hazards, and struck by hazards while cleaning out metal debris and greasing the chuck in that the employer did not ensure that steps (d)(1) through (d)(6) were followed by each authorized employee by isolating each energy source such as electrical.  b) On or about May 19, 2025, employees operating the Kingston Lathe Work Center 52111R were exposed to caught-in hazards and struck by hazards while performing servicing and/or setup such as cleaning the lathe, lubricating parts of the machine, and setup and removal of the parts being worked. in that the employer did not ensure that steps (d)(1) through (d)(6) were followed by each authorized employee by isolating each energy source such as electrical.

Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).  a)  On or about May 15, 2025, the employer did not ensure that employees operating the Komatsu 25 propane-powered sit-down forklift had completed the required training prior to being allowed to operate the forklift.

29 CFR  1910.184(h)(5): Removal from service. Natural and synthetic fiber rope slings shall be immediately removed from service if any of the following conditions are present:  (i) Abnormal wear. (iii)Broken or cut fibers. (iv)Variations in the size or roundness of strands. (v) Discoloration or rotting.  a) On or about May 19, 2025, the employer did not ensure that damaged fiber rope slings were removed from service. Slings with abnormal wear, broken or cut fibers, variations in strand size or roundness, and discoloration or rotting were used with overhead cranes to move loads weighing up to 2,000 pounds, exposing employees to struck-by and crushing hazards.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  a) On or about May 19, 2025, the employer did not ensure that guarding was provided on the Kingston Lathe, Work Center 52111R, at the chuck and at the tool post to protect operators from struck-by and caught-in hazards.
