29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee. Location - Roof On or about 5/20/2025, employees were exposed to fall hazards greater than 17 feet to the ground below when using a personal fall arrest system consisting of a Miller RA25-1/unidentifiable reusable roof anchor and a that was improperly installed, in that, only a total of two 16d nails (two nails on one side of the metal flap) were used to secure the anchor to the roofs surface.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. Location - Front of the house On or about 5/20/2025, employees used improperly set up aluminum extension ladders that did not extend 3 feet above the upper landing, exposing them to fall hazards.
