On or about May 15th, 2025, employees engaged in cleaning and maintenance related activities at 1852 10th Ave, Columbus, Nebraska 68601, were exposed to electrical and pneumatic energy sources  when accessing machine 850 #1 in that the employer was allowing authorized LOTO operators access and perform work inside the 850#1 without receiving authorized LOTO training.

On or about May 20th, 2025, employees engaged in manufacturing operations located at 1852 10th Ave, Columbus, Nebraska 68601, were exposed to fire and electrical hazards in that a damaged flexible cord was plugged into the VIBRO BLOCK FEEDER.  The flexible cord outer sheathing was damaged and was exposing energized 120V.

29 CFR  1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  On or about May 20th, 2025, employees engaged in manufacturing operations located at 1852 10th Ave, Columbus, Nebraska 68601, were exposed to electrical and fire hazards in that the employer was not inspecting flexible cords.  The employer was using a damaged flexible cord that was plugged into the VIBRO BLOCK FEEDER.

On or about May 20th, 2025, employees engaged in manufacturing operations located at 1852 10th Ave, Columbus, Nebraska 68601, were exposed to fire and electrical hazards in that the conduit that was ran to the oven blowers on the back of the curing oven  was not sealed or plugged at the outer end.  The Conduit was hanging freely and was exposing energized 120V conductors.

29 CFR  1910.305(g)(2)(iii):Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  On or about May 20th, 2025, employees engaged in manufacturing operations located at 1852 10th Avenue, Columbus, Nebraska 68601, were exposed to fire and electrical hazards in that electrical cabinet that supplies power to the conveyor motor on machine 850 #1 did not contain a device or fitting that would prevent the energized cable from damage.  The cable inserted into the cabinet had damage to the outer sheathing and was exposing energized 230V.
