OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards from falling loads and/or lift parts:  a) Bay 3 , 825 South 1st Street, Fulton, NY 13069, on or about 5/20/2025, a two-post surface mounted hydraulically driven mechanical Challenger Lift, did not pass inspection and was operated with safety hazards including, but not limited to, a missing left front adaptor pad, left front arm restraint missing teeth, and a cracked hydraulic oil reservoir (Model: CL9, SN 900986).  b) Bay 5 , 825 South 1st Street, Fulton, NY 13069, on or about 5/20/2025, a two-post surface mounted hydraulically driven mechanical Rotary Lift, did not pass inspection and was operated with safety hazards including, but not limited to, slider blocks thin and worn, cables maxed out and starting to unwind, both hydraulic oil cylinders leaking from seals and both hoses seeping oil (Model: SPOA9-100, SN AIB96C0894).  c) Bay 6 , 825 South 1st Street, Fulton, NY 13069, on or about 5/20/2025, a two-post surface mounted hydraulically driven mechanical Rotary Lift, did not pass inspection and was operated with safety hazards including, but not limited to, cables are extremely frayed (Model: RTP9000M, SN VVT15H0001).
