29 CFR  1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  (a)  On or about May 15, 2025, employees were attempting to unjam the Amada Turret Punch Press (Model No. PEGA-204040 and Serial No. P2440068).  The employer did not ensure that the turret press was locked out, energy sources blocked or dissipated prior to servicing the turret press, exposing employees to struck by and crushing hazards from mechanical, hydraulic and gravity energy.

29 CFR  1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices:  (a)  On or about May 15, 2025, employees were attempting to unjam the Amada Turret Punch Press (Model No. PEGA-204040 and Serial No. P2440068). The employer did not ensure that all potentially hazardous store energy (gravity) was restrained, in-that the top plate of the turret punch press was not blocked.  This exposed employees to struck by and crushing hazards.

29 CFR  1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a)  On or about May 21, 2025, employees were exposed to amputation hazards while working on and around Amada Turret Punch Press (Model No. PEGA-204040 and Serial No. P2440068). The employer did not ensure employees were protected from amputation hazards created by the point of operation.  The interlock switch on the access door (tool change panel) to the turret punch press had been zip tied to allow the access door to be opened while the machine was still in operation.    (b)  On or about May 21, 2025, employees were exposed to amputation hazards while working on and around Amada Turret Punch Press (Model No. COMA-505072 and Serial No. C57146), in-that the employer did not ensure employees were protected from amputation hazards created by the point of operation.  The access door (tool change panel) to the turret punch press was not interlocked or secured in a way to limit access while the machine was still in operation.    (c)  On or about May 21, 2025, employees were exposed to amputation hazards while working on and around the Cincinnati 350 Ton Press Brake (Serial No. 44776), in-that the employer did not ensure employees were protected from amputation hazards created by the point of operation. The light curtains protecting employees from the point of operation had been unplugged and no other means of guarding had been implemented.    (d)  On or about May 21, 2025, employees were exposed to amputation hazards while working on and around  the Cincinnati 135 Ton Press Brake (Serial No. 43861), in-that the employer did not ensure employees were protected from amputation hazards created by the point of operation. The light curtains protecting employees from the point of operation had tape placed over the censors, causing approximately 4" of the light curtain to be ineffective,  and no other means of guarding had been implemented.
