(a)  On or about May 22, 2025, the employer did not ensure that employees used personal protective equipment identified in the workplace hazard assessments, including eye and face protection, chemical-resistant gloves, abrasion-resistant gloves, protective sleeves, and chemical protective aprons. As a result, employees engaged in grinding, polishing, and working over dip tanks containing strong acids, bases, and toxic chemicals were exposed to hazards of flying particles, airborne dust, and chemical splashes that could cause serious injuries.

(a)  On or about May 22 and June 17, 2025, the employer did not require employees to wear eye and face protection, such as but not limited to safety glasses, goggles, and a face shield, when plating parts in the tank room, where the employees were exposed to splash hazards from dip tanks of chemicals containing hydrochloric acid(severe skin and eye burns), sulfuric acid(corrosive, severe skin and eye burns), sodium cyanide (severe skin and eye burns, damage to organs through prolonged exposure), potassium cyanide(corrosive, fatal if swallowed, severe skin and eye irritation), copper cyanide(fatal if swallowed, in contact with skin or inhaled), nickel sulfate(cancer, allergic skin reaction, damage to organs through prolonged or repeated exposure), sodium hydroxide(corrosive, severe skin burns and eye damage), and other plating chemicals.   (b)  On or about May 22 and June 17, 2025, the employer did not require employees to wear eye and face protection, such as but not limited to safety glasses, goggles, and a face shield, when the employees were grinding, polishing and buffing metal parts, which created fine dust particles in the air that could irritate eyes.

29 CFR  1910.134(k)(6): The basic advisory information on respirators, as presented in Appendix D of this section, shall be provided by the employer in any written or oral format, to employees who wear respirators when such use is not required by this section or by the employer.   (a)  On or about May 22, 2025, the employer did not provide employees with the advisory information contained in Appendix D of the OSHA Respiratory Protection Standard when issuing 3M 8511 N95 filtering facepiece respirators for voluntary use.  As a result, employees used respirators without being informed of the limitations, proper use, and maintenance requirements necessary to ensure adequate protection.
