29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):   (a)Dell Rapids Custom Trailers, Inc. at 47138 246th Street, Dell Rapids, SD, 57022:  On or about and at times prior to May 21, 2025, the employer did not establish a program consisting of energy control procedures and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative, including but not limited to the following (as required by the standard):   (1) Saws (2) Drill Presses (3) Iron Workers (4) Shearers	 (5) Brake Presses  Abatement Note: As part of an energy-control program, employers must:  (1) Establish energy-control procedures for removing the energy supply from machines and for putting appropriate lockout or tagout devices on the energy-isolating devices to prevent unexpected reenergization. When appropriate, the procedure also must address stored or potentially reaccumulated energy.   (2) Train employees on the energy-control program, including the safe application, use, and removal of energy controls; and   (3) Inspect these procedures periodically (at least annually) to ensure that they are being followed and that they remain effective in preventing employee exposure to hazardous energy.   The energy-control procedures must outline the scope, purpose, authorization, rules, and techniques that employees will use to control hazardous energy sources, as well as the means that will be used to enforce compliance. These procedures must provide employees at least the following information:   (1) A statement on how to use the procedures;  (2) Specific procedural steps to shut down, isolate, block, and secure machines;   (3) Specific steps designating the safe placement, removal, and transfer of lockout/tagout devices and identifying who has responsibility for the lockout/tagout devices; and   (4) Specific requirements for testing machines to determine and verify the effectiveness of lockout devices, tagout devices, and other energy-control measures.

29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    (a)Dell Rapids Custom Trailers, Inc. at 47138 246th Street, Dell Rapids, SD, 57022:  On or about and at times prior to May 21, 2025, the employer did not ensure that forklift operators, were competent to operate the  Doosan, Model 630E-7, powered industrial truck safely by a successful completion of a training program prior to operating.  This condition exposed employees to struck-by hazards associated with improper forklift operations.  Abatement Note: Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)Dell Rapids Custom Trailers, Inc. at 47138 246th Street, Dell Rapids, SD, 57022:  On or about and at times prior to May 21, 2025, the employer did not ensure that the Scotchman Ironworker, located in the Flooring Area, Model No. FI 51, Serial No.1041M1096, was properly guarded at all points of operation. This condition exposed employees to caught-in, crushing, and amputation hazards.  (b)Dell Rapids Custom Trailers, Inc. at 47138 246th Street, Dell Rapids, SD, 57022:  On or about and at times prior to May 21, 2025, the employer did not ensure that the Piranha Iron Worker located in the Fabrication Area, was properly guarded at all points of operation. This condition exposed employees to caught-in, crushing, and amputation hazards.  (c)Dell Rapids Custom Trailers, Inc. at 47138 246th Street, Dell Rapids, SD, 57022:  On or about and at times prior to May 21, 2025, the employer did not ensure that all portions of the saw blade are enclosed or guarded on the Enco horizontal bandsaw, Model No. MSC 009195522., Serial No. 94458, located in the Fabrication Area,  in that employees are exposed to 12 inches  of the saw blade when using the saw to cut / fabricate  metal for trailer parts.  This condition exposed employees to pinching, laceration, and amputation hazards.

This condition exposed employees to crushing and amputation hazards.  (b)Dell Rapids Custom Trailers, Inc. at 47138 246th Street, Dell Rapids, SD, 57022:  On or about and at times prior to May 21, 2025, the employer did not ensure that the Chicago Apron Brake, Model No. HSB 1012, Serial No. 328275T, located in the Cold  Storage Area, was guarded at the point of operation. This condition exposed employees to pinching, crushing, and amputation hazards.
