OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees who were exposed to fire and explosion hazards: (a) On May 15, 2025, on the Yellowstone West Pad, in the vicinity of CR45 & 24th Street NW in Watford City, North Dakota, an employee was exposed to fire and explosion hazards while attempting to clear a freeze plug from the Yellowstone - 8H well natural gas artificial lift system where the natural gas was vented directly into the space and was ignited. Note: Among other methods, feasible and acceptable abatement methods to correct this hazard include but are not limited to: 1. Follow API Standard 521 Section 5.8.10.1 Vent Stacks - Where the atmospheric vent handles combustible vapors, the outlet from the vent should be elevated approximately 3 m (10 ft) above any adjacent equipment, building, chimney, or other structure. 2. Follow NFPA 55 - Compressed Gases and Cryogenic Fluids Code Section 6 Building-Related Controls; Sub-Section 6.14 Vent Pipe Termination: The termination point for piped vent systems servicing cylinders, containers, tanks and gas systems used for the purpose of operational or emergency venting shall be located to prevent impingement exposure on the system served and to minimize the effects of high temperature thermal radiation or the effects of contact with the gas from the escaping plume to the supply system, personnel, adjacent structures, and ignition sources.
