29 CFR 1904.29(a):The employer did not use an OSHA 300, 301 and 300A Form or equivalent. A.) B and H Delivery Inc., On or about 5/22/25 and times prior thereto: The company did not maintain OSHA 300 Log of Work-Related Injuries and Illnesses, 301 Injury and Illness Incident Report, and 300A Summary of Work-Related Injuries and Illnesses or equivalent forms, for recordable injuries and illnesses. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

B and H Delivery Inc., On or about 5/22/25 and times prior thereto: Employees conducted work operations including but not limited to, vehicle maintenance and repairs. The assessment of the workplace to determine what Personal Protective Equipment (PPE) is required for employees was not documented and certified.
