29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. (a) 2816 Zane A Miller Drive, Waynesboro, PA - On or about May 12, 2025, the employer did not develop a written workplace hazard assessment to identify and document hazards (or potential hazards) including, but not limited to, those associated with flying particles and/or injurious light radiation within the evaluated workplace that would necessitate the use of personal protective equipment (PPE). Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
