OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employer utilized damaged storage rack systems, exposing employees to struck-by and crushing hazards. a. Located at 5653 Creekside Pkwy Lockbourne, OH 43137: On or about May 23, 2025, and times prior, the employer failed to ensure that the storage racks located at or near 08-B-105, 08-A-305, 09-B-195, and 118-A-130 were free from column deflection damage ranging from 1/2 inch to approximately 6 inches. b. Located at 5653 Creekside Pkwy Lockbourne, OH 43137: On or about May 23, 2025, and times prior, the employer failed to ensure that the storage racks located at or near 09-B-195, and 118-A-130 were free of loose/broken base plates of approximately 2 inches.
