29 CFR  1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  a.  Located at 774 Leslie Drive, Plain City, OH 43064:  On or about May 22, 2025, employees installing a stone facade on a two-story residence were not provided with an adequate ladder to access a scaffold platform set at approximately 6' from the ground. An approximately 3' step ladder was used to access the work platform.

29 CFR  1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder.   a.  Located at 774 Leslie Drive, Plain City, OH 43064:  On or about May 22, 2025, employees used an approximately 3' tall stepladder to access a scaffold work platform set at a height of approximately 6' which required employees to climb from the top of the ladder up onto the platform.
