On or about May 23rd, 2025, employees engaged in residential framing operations on the job site located at 8125 South 198th Ave, Gretna, Nebraska 68028, were exposed to fall hazards in that employees were observed working at heights greater than 21 feet 7 inches without any form of fall protection in place.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee who might be exposed to fall hazards: On or about May 23rd, 2025, employees engaged in residential framing operations on the job site located at 8125 South 198th Ave, Gretna, Nebraska 68028, were exposed to fall hazards in that the employer did not provide the employees training on information to include but not limited to: the hazards associated with working at elevations over 6 feet, inspecting fall protection equipment before use and recognition of potentially hazardous fall conditions prior to exposing the employees to heights over 21 feet 7 inches.
