29 CFR  1926.451(a)(6): Scaffolds were not designed by a qualified person:  Location: 604 Ashford Ave, Ardsley, NY  a) On or about May 15, 2025, an employee doing framing work from a makeshift scaffold that was constructed by placing an aluminum stage on two horizontal 2"x6" planks of reclaimed wood screwed to interior framing. The ends of those 2"x6" planks extending to the building exterior were screwed to two vertical 2"x6" planks of reclaimed wood. The scaffold was not designed and constructed by a qualified person exposing employees to fall hazards of approximately 10' to the ground below.   b) On or about July 2, 2025, employees were working on siding and soffits approximately 18' high from a makeshift scaffold that was constructed by placing an aluminum stage between a ladder with a ladder jack and a pump jack pole. The scaffold was not designed and constructed by a qualified person exposing employees to fall hazards of approximately 18' to the ground below.

The uprights did not bear on base plates, mud sills or other adequately firm foundation.  b) On or about July 2, 2025, employees performing siding and soffit work on the second floor of a residential building from a scaffold with a pole that was placed on soil. The pole did not bear on base plates, mud sills or other adequately firm foundation.

The scaffold was not designed and constructed by a qualified person exposing employees to fall hazards of approximately 18' to the ground below.  Barrera's Contracting Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 1678691, citation number 1, item number 4 and was affirmed as a final order on September 4, 2024, with respect to a workplace located at 5 Howell Place, Eastchester, NY.
