29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) New construction located at 3618 LaVista Road Decatur, GA 30033, Lot 2; on or about 05/07/2025: The employer exposed employees to struck-by hazards during framing activities in that head protection was not provided where work was being performed overhead such as but not limited to placement of floor joists.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New construction located at 3618 LaVista Road Decatur, GA 30033, Lot 2; on or about 05/07/2025: The employer exposed employees to approximately 10-feet, 1-inch fall hazards from the top plate to the lower level while positioning floor joists in that a fall protection system was not provided.  b) New construction located at 3618 LaVista Road Decatur, GA 30033, Lot 2; on or about 05/07/2025: The employer exposed employees to  approximately 10-feet, 8-inches from the first floor to the basement while walking at or around the floor hole in that a fall protection system was not provided.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) New construction located at 3618 LaVista Road Decatur, GA 30033, Lot 2; on or about 05/07/2025: The employer exposed employees to approximately 10-feet, 1-inch fall hazards from the top plate to the lower level while accessing and egressing the top plate with a 5-foot Werner step ladder and a 7-foot Little giant multi-position ladder in that the ladders did not extend 3-feet above the top plate.
