29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids, caustic liquids, or chemical gases or vapors and at times prior thereto, workers were exposed to a struck by hazard while cutting siding with a saw, without the use and wear of protective eyewear:  a.  At a temporary worksite, located at the east side of 5539 Limber Mill Lot 431 Jacksonville FL:  On or about May 27, 2025 the employer exposed employees to struck-by eye injury hazards from flying debris, in that protective eyewear use was not enforced by the employer while employees used electrical saws, metal hammers, and pneumatic nail guns for siding installation.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.  At a temporary worksite, located at the east side of 5539 Limber Mill Lot 431 Jacksonville FL:  On or about May 27, 2025, the employer was exposing an employee to a 10 -11 foot fall hazard, in that the employees were not tied off when installing the siding.  a.  At a temporary worksite, located at west side of 5539 Limber Mill Lot 431 Jacksonville FL:  On or about May 27, 2025, the employer was exposing an employee to a 11- 12 foot fall hazard, in that the employees were not tied off when installing the siding.

29 CFR  1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.   a.  At a temporary worksite, located at west side of 5539 Limber Mill Lot 431 Jacksonville FL:  On or about May 27, 2025 the employer did not provide fall protection training that included the use and operation of personal fall arrest systems to roofing employees exposed to fall hazards up to 10 -12 feet to the ground below while installing the siding at a residential house.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a.  At a temporary worksite, located at 5539 Limber Mill Lot 431 Jacksonville FL: On or about May 27, 2025,  employees were exposed to a fall hazard while using a Werner 13 ft. Aluminum Extension Ladder not extending at least 3 feet above the landing.
