29 CFR  1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee:   On or about May 27, 2025, on the front side of the site, an employee was anchored to the mast of a telehandler that was being used to elevate a wooden pallet greater than 6 feet.

29 CFR  1926.602(c)(1)(viii)(A):The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel:  On or about May 27, 2025, on the front side of the site, an employee was welding while standing on a wooden pallet that was not secured to the forks and being elevated by a JLG Telehandler Model 6042.
