29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a) On or about May 27, 2025, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns, in that protective eyewear was not provided to employees engaged in framing activities while operating these tools at 130 PSI.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a) Portico: On or about May 27, 2025, the employer exposed employees to fall hazards of approximately 15 feet to the lower level, in that a fall protection system was not provided to employees engaged in framing activities.  b) Second level: On or about May 27, 2025,  and at times prior, the employer exposed employees to fall hazards of approximately 18 feet to the lower level, in that a fall protection system was not provided to employees engaged in framing while working with an unprotected side or edge.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Second floor: On or about May 27, 2025, the employer exposed employees to fall hazards of approximately 10 feet 5 inches to the lower level, in that the ladder provided did not have siderails which extended at least 3 feet past the landing surface and was not secured to allow for safe access.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:   a) On or about May 27, 2025, the employer failed to inspect all electrical extension cords used to provide power to electric tools which employees engaged in framing operations were using, in that the yellow extension cord powering the circular and reciprocating saws was damaged. An inspection of these cords was not performed prior to the shift as these cords were not repaired or removed from service.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a) On or about May 27, 2025, the employer exposed employees to 120V electrical shock hazards while operating reciprocating and circular saws, impact drills, and battery chargers, in that the yellow extension cord was missing the grounding pin.
