Employee(s) were performing roof remove-repair-replace activities on a residential roof 22-feet above ground without sufficient fall protection equipment resulting in a serious Employee injury.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface, and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access.  83 Foxcroft Road, WEST HARTFORD, CT 06119: On or about May 20, 2025, Employer allowed Employees conducting roofing activities on a residential 10-12 pitch roof approximately 21-feet above ground, with an improperly positioned portable ladder that did not extend at least 3-feet above the upper landing surface used to gain access.
