1926.454(c)(3)  The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency:  Thompson Building, Penn State University - On or about May 27, 2025, a mid-rail on the scaffold in use did not withstand the required applied force and exposed a worker to a fall in excess of 30 feet to grade.  Employees lacked the skill or understanding needed for the safe erection of the scaffolding in that they did not understand the applied forces to the guardrail system and /or the proper techniques  to install the guardrail system.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
