Employees were installing roof and flooring materials on the top floor of a new construction, high-rise commercial building, over 6-stories high and working within a foot of the floors unprotected edge without properly utilizing any fall protection systems.

Safety harnesses being worn by Employees while working within the platforms of an extendable boom-aerial lift were not secured to the platforms anchor points or a retractable lanyard device. The Fernando Molina Construction LLC, now Fernando JJ Carpentry LLC, was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1788979, citation number 1, item number 1a, and was affirmed as a final order on October 30, 2024, with respect to a workplace located at 104 Episcopal Road, Berlin, CT 06037.
