29 CFR 1926.502(d)(15)(i):  Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two:   On or about May 28, 2025, employees performing roofing work were exposed to fall hazards of over 6 feet and the employer did not ensure the Guardian Temper roof peak anchors maintained a safety factor of at least two:  a. Two employees working on the south-facing roof area were attached to a single anchor designed for a maximum of one (1) personal fall arrest system.  b. Two employees working on the south-facing roof area were attached to a single anchor secured to the roof with only one nail on each side.

Two employees working on the south-facing roof area were attached to a single anchor designed for a maximum of one (1) personal fall arrest system.  b. Two employees working on the south-facing roof area were attached to a single anchor secured to the roof with only one nail on each side.
