29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts: On or about May 20, 2025, an employee was injured while spraying Blaster Industrial Graphite Dry Lubricant and applying grease to the trunnion wheels as part of their daily tasks in the East side of the scrap yard, outside and North of the Non-Ferrous Return Plant (NFRP area). The employee's hand was caught in between the rotating rollers and tumblers of the TR-850 Trommel machine. The hand laceration occurred due to no guarding system in place to cover the rotating the rollers and tumblers.

29 CFR 1910.219(f)(3): Sprocket wheels and chains that were seven (7) or less feet from the floor or platform were not enclosed: Employees spraying Blaster Industrial Graphite Dry Lubricant and applying grease to the trunnion wheels as part of their daily tasks in the East side of the scrap yard, outside and North of the Non-Ferrous Return Plant (NFRP area) were exposed to amputation and laceration in that the moving chain and sprockets next to the West side of the rollers and tumblers of the TR-850 Trommel machine were not guarded.
