29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Crushproof Tubing Company located at 100 North Street, McComb, Ohio, 45858: On or about May 29, 2025, the employer failed to ensure that the pinch point created between the sliding pole and plunger of Corder and #11 was adequately guarded. In this condition, employees working on or near the plunger of Corder #11 were exposed to caught in hazards.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a) Crushproof Tubing Co. Inc. located at 100 North Street, McComb, Ohio, 45848: On or about May 29, 2025, the employer did not ensure that energized conductors (110V) near the plunger on Corder #8 were adequately closed. Employees performing operational duties on or near these conductors on Corder #8 were exposed to electrical shock hazards.
