29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  (a) On or about May 29, 2025, the employer did not ensure that frequent and regular jobsite inspection were conducted to ensure employees were protected from hazards associated with their work. Hazards identified on the jobsite include but not limited to lack of PPE, incorrect setup of an extension ladder, and no fall protection equipment installed.

(a) On or about May 29, 2025, the employer did not ensure that employees performing re-roofing on two-story residential property were protected from falling to the ground below. Exposing employees to a fall hazard of approximately 17 feet.  (b) On or about May 29, 2025, the employer did not ensure that employees performing re-roofing on the one-story attached garage roof were protected from falling to the ground below. Exposing employees to a fall hazard of approximately 8 feet.
