29 CFR  1903.19(d)(1):The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a)  On or about September 2, 2025, the employer failed to submit documentation and certification of corrective actions, for  Serious citation 1, items 1, and 2 issued on June 25, 2025 with an abatement due date of July 22, 2025.   b)  On or about September 2, 2025, the employer failed to submit documentation and certification of corrective actions, for  Repeat - Serious citation 2, item 1,  issued on June 25, 2025 with an abatement due date of July 22, 2025.

29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) At the jobsite located at 19316 Gulf Blvd, Indian Shores, Florida: On or about 05/29/2025; the employer exposed employees to struck-by hazards, in that, employees were using nail guns without wearing eye protection.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) At the jobsite located at 19316 Gulf Blvd, Indian Shores, Florida: On or about 05/29/2025; the employer exposed employees to fall hazards, in that, fall protection was not utilized by employees while replacing shingles on a roof that had a fall height of approximately 8 ft. 6 in. to the ground with a 4:12 pitched (vertical to horizontal) roof.  The L & E REMODELING CORP was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1608203, citation number 1, item number 1 and was affirmed as a final order on 12/20/2022, with respect to a workplace located at 13447/13445, located at Ocean Mist Drive, Jacksonville, FL 32258.
