29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:  On or about May 29, 2025, during the installation of stucco material on the exterior of a two-story commercial building located at 9701 Brookpark Road, Parma, Ohio, the employer failed to ensure the working level(s) of a fabricated frame type scaffold were fully planked or decked between the front uprights and guardrail supports.  Employees were working on the third (uppermost) level of the scaffold, with a scaffold frame width of 5 feet (60 inches) with only one or two 19 inch wide scaffold planks to make up the platform, exposing them to an approximately 20 foot fall.     Complete Stucco Inc., LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1) which was contained in OSHA inspection number 1583788, citation number 1, item number 1-a, and was affirmed as a final order on 4/27/2022, with respect to a workplace located at 3775 Matisse Court, Pepper Pike, Ohio.

29 CFR  1926.451(e)(1):When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  On or about May 29, 2025, during the installation of stucco material on the exterior of a two-story commercial building located at 9701 Brookpark Road, Parma, Ohio, the employer failed to ensure employees working on a three level high fabricated frame type scaffold, with platforms more than 2 feet above or below a point of access were provided with a portable ladder, attachable ladder, or other safe means to access or egress between the scaffold platforms.  The first level scaffold platform was located approximately 7 feet above the ground.  The second level platform was located approximately 6 feet above the first level platform.  The third level platform was located approximately 7 feet above the second level platform.     Complete Stucco Inc., LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1) which was contained in OSHA inspection number 1583788, citation number 1, item number 1-d, and was affirmed as a final order on 4/27/2022, with respect to a workplace located at 3775 Matisse Court, Pepper Pike, Ohio.

29 CFR  1926.451(g)(1):Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold.   On or about May 29, 2025, during the installation of stucco material on the exterior of a two-story commercial building located at 9701 Brookpark Road, Parma, Ohio, the employer failed to ensure employees working on the third level platform of a fabricated frame type scaffold, at a height of approximately 20 feet above the ground, were protected from falling to the ground (lower level) by a guardrail or other fall protection system.    Complete Stucco Inc., LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1) which was contained in OSHA inspection number 1583788, citation number 1, item number 1-e, and was affirmed as a final order on 4/27/2022, with respect to a workplace located at 3775 Matisse Court, Pepper Pike, Ohio.
