29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:  a) Job site, near 46 South Street, Sewickley, PA: On or about May 29, 2025 during roofing work, the employer did not ensure an employee used appropriate fall protection systems/devices where a fall hazard of approximately 20 feet existed.  Omar Ramirez Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1491508, citation number 1, item number 1, and was affirmed as a final order on November, 20, 2020, with respect to a workplace located at 180 Wellington Circle, McMurray, PA.

29 CFR 1926.20(b)(2): A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons:  a) Job site, near 46 South Street, Sewickley, PA: On or about May 29, 2025 during roofing work, the employer did not ensure a system of hazard recognition and corrections was implemented at the job site. Deficiencies noted during the inspection included, but were not limited to, ladders not being properly extended above the landing and fall protection not being used where needed.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, near 46 South Street, Sewickley, PA: On or about May 29, 2025 during roofing work, the employer did not ensure the ladder used to get up to/down from the roof was properly set up in that it was determined that it only extended approximately 30 inches above the roofline.
