29 CFR  1903.19(d)(1):The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a)  The employer failed to submit documentation of abatement within 10 days of the abatement date for Citation 1 Item 1, Citation 1 Item 2, and Citation 1 Item 3.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about May 30, 2025, employee installing shingles on the roof of a multi-level residential building, at a height greater than six feet, were not protected from falls to the lower level.  AFAV Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1733891, citation number 1, item number 2 and was affirmed as a final order on October 07, 2024, with respect to a workplace located at 2606 Commercial Ave., Madison, WI 53704.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about May 30, 2025, employees shingling a multi-level residential roof while using pneumatic nail machines were doing so without the use of eye protection exposing the employees to eye or face hazards from flying particles.  AFAV Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1733891, citation number 1, item number 1 and was affirmed as a final order on October 07, 2024, with respect to a workplace located at 2606 Commercial Ave., Madison, WI 53704.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about May 30, 2025, the employer did not ensure employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work in that all employees were working from heights greater than six feet without fall protection. 29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about May 30, 2025, employees reinstalling shingles on the roof of a multi-story residential building during reroofing and did not have the ladder used for accessing the roof at least 3 feet above the landing surface.
