29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a)Hot Cuisine Room: Procedures were not utilized for cleaning the Hughes Company, Inc. Blancher, (large blancher) Model No. 002-2375D, Serial Number 07-22282, exposing employees to a rotating auger, on or about 5/22/25. b)Burrito Room: Procedures were not utilized for the L28 C-Pack #1 Conveyor, Serial Number 10400 during set up and cleaning, exposing employees to ingoing nip point on or about 10/8/25. c) Hot Cuisine Room: Procedures were not developed, documented and utilized for the Hughes Company, Inc. Blancher, Model No. 002-2378D, Serial Number 07-22283 (the small blancher) when cleaning this machine, exposing the employees to a rotating auger. On 5/22/25 an employee sustained fatal injuries while cleaning the machine. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
