On or about June 6, 2025 three employees working on the 8-pitch roof and awning of a 1.5 story residence were not protected from falling by a guardrail system, safety nets, or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. 167 West Washington Street Bristol, CT. On or about June 2, 2025 two portable ladders used to access a roof and awning did not extend at least three feet above the upper landing and were not secured at the top to a rigid support.
