29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. a) On or about June 2, 2025, at an establishment located at 2580 West 9th Avenue in Oshkosh, Wisconsin, the project manager of Right Choice Improvements was present at an active jobsite and observed employees of Right Choice Improvements being exposed to fall hazards of over 9 feet 3 inches and failed to take prompt corrective measures to eliminate the hazards. b) On or about June 2, 2025, at an establishment located at 2580 West 9th Avenue in Oshkosh, Wisconsin, the project manager of Right Choice Improvements was present at an active jobsite and observed sub-contracted employees of Octavio's Home Improvement LLC being exposed to fall hazards of over 11 feet and failed to take prompt corrective measures to eliminate the hazards. c) On or about June 2, 2025, at an establishment located at 2580 West 9th Avenue in Oshkosh, Wisconsin, the project manager of Right Choice Improvements was present at an active jobsite and observed sub-contracted employees of MBE Roofing Specialist LLC being exposed to fall hazards of over 9 feet 3 inches and failed to take prompt corrective measures to eliminate the hazards. d) On or about June 2, 2025, at an establishment located at 2580 West 9th Avenue in Oshkosh, Wisconsin, the project manager of Right Choice Improvements was present at an active jobsite and observed sub-contracted employees of A&A Roofing & Siding LLC being exposed to fall hazards of over 13 feet 4 inches and failed to take prompt corrective measures to eliminate the hazards.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a) On or about June 2, 2025, at an establishment located at 2580 West 9th Avenue in Oshkosh, Wisconsin, an employee of Sagacious Solutions Inc. dba Right Choice Improvements was exposed to fall hazards of over 9 feet without utilizing any form of fall protection while engaged in commercial roofing activities. b) On or about June 2, 2025, at an establishment located at 2580 West 9th Avenue in Oshkosh, Wisconsin, employees Octavio's Home Improvement LLC, working as subcontractors of Sagacious Solutions Inc. dba Right Choice Improvements were exposed to fall hazards of over 11 feet without utilizing any form of fall protection while engaged in commercial roofing activities. c) On or about June 2, 2025, at an establishment located at 2580 West 9th Avenue in Oshkosh, Wisconsin, employees MBE Roofing Specialist LLC , working as subcontractors of Sagacious Solutions Inc. dba Right Choice Improvements were exposed to fall hazards of over 9 feet without utilizing any form of fall protection while engaged in commercial roofing activities. d) On or about June 2, 2025, at an establishment located at 2580 West 9th Avenue in Oshkosh, Wisconsin, employees A&A Roofing & Siding LLC , working as subcontractors of Sagacious Solutions Inc. dba Right Choice Improvements were exposed to fall hazards of over 11 feet without utilizing any form of fall protection while engaged in commercial roofing activities.
