29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a) On or about June 6, 2025, at a worksite located at 2619 Wisconsin Ave NW, Washington, DC, employees were observed working on scaffold system in which the working platform was not fully planked, exposing them to an estimated 25-foot fall while conducting masonry work.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR  1926.451(b)(5)(i):The end of a platform 10 feet or less in length, extended over its support more than 12 inches:  a) On or about June 6, 2025, at a worksite located at 2619 Wisconsin Ave NW, Washington, DC. The employer had installed the planks of the working platform an estimated eighteen inches beyond the support creating a cantilever effect and exposing the employees to a potential fall hazard of 25 feet while conducting their assigned masonry work. A competent person should have been aware of these hazards and know how to abate them before having employees working on the scaffold.
