Employees were using an extension ladder to access the roof, and the employer did not ensure it extended at least 3 feet above the upper landing surface.

Aguirre Roofing L.L.C. was previously cited for a violation of this occupational safety and health standard or its equivalent standard  1926.501(b)(13), which was contained in OSHA inspection number 1501019, citation number1, item number 1 and was affirmed as a final order on 12-15-2020, with respect to a workplace located at 8131 Wyatt, Broadview Heights, Ohio 44147.
