29 CFR  1910.95(g)(1):The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  a. A machine operator working in the hose manufacturing area was exposed to a noise dose of 108.7%, or an equivalent dBA of 90.6 dBA, which was capable of causing permanent hearing loss. The sampling was performed for 417 minutes during one shift on June 4, 2025. The employer did not provide baseline audiograms for employees that performed this task.

29 CFR  1910.95(k)(1):The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k). The employer did not institute a training program and ensure employee participation in the program:  a. A machine operator working in the hose manufacturing area was exposed to a noise dose of 108.7%, or an equivalent dBA of 90.6 dBA, which was capable of causing permanent hearing loss. The sampling was performed for 417 minutes during one shift on June 4, 2025. The employer did not ensure all employees working in the hose manufacturing area received training on the effects of noise on hearing, purpose of hearing protectors, fitting, use, care and purpose of audiometric testing prior to performing tasks in this area.

29 CFR  1910.134(e)(1):The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  a.  In the welding booth area: On or about June 4, 2025 and at times prior, the employer exposed employees to respiratory hazards, in that that the employer allowed the use of a respirator by employees without first providing a medical evaluation to determine if the employees were able to wear a respirator or not.
